Procedures
Initial Registration
At the first visit, the parent/guardian will be given a Child Information Card which includes the following information that needs to be completed: Child’s Name, Parent’s Names, Address, Phone Number, Birthday, Doctor, Allergies, Dietary Restrictions, Age/Department, Name of person(s) other than parent to whom the child may be released, Yes or No CWMC has permission to authorize emergency medical treatment, Parent/Guardian signature and date.

Nursery – 3 year old Procedures
Weekly Sign-In
Each week, including that first visit, the parent/guardian will sign the child in on the attendance ‘Sign In Sheet’, which contains the following information: Child’s Age, Name, Parent/Guardian name, which service(s) they will be attending, and a comment area to record known allergies, special instructions, etc.
We utilize a pager system to allow parents to feel secure knowing they can be reached anywhere in the building at a moment’s notice. Below are the procedures to keep in mind as you check your child into the age-appropriate room. The pager MUST be turned in or checked off in order to pick up your child. This is for your safety, the safe of your child(ren), and the safety of the CM Volunteers.
Check In Procedure
If you only have one child or if this is the first child you are checking in:
1) Sign in your child on the sign-in sheet.
2) You will be given a pager with a corresponding number.
3) Write down that number on the sign-in sheet next to your child’s name.
4) The pager will vibrate if you are needed, so please keep it on your body somewhere.
If you are checking in children in different rooms:
1) Sign in your child on the sign-in sheet.
2) Write down the number of your previously assigned pager from the other room on the sign-in sheet.
3) Again, the pager will vibrate if you are needed.
Check Out Procedure
1) When you return to pick up your child, turn in your pager to the attendant.
2) If you have other children to pick up, and the pager belongs to another room, return it to that room.
3) NOTE: Whichever room the pager was checked out from needs to be the last room that you pick up your child from.
4 Year Olds – Kindergarten Procedures

Each week, including that first visit, the parent/guardian will sign the child in on the attendance ‘Sign In Sheet’, which contains the following information: Child’s Age, Name, Parent/Guardian name, a comment area to record known allergies, etc., and where the parent/guardian will be (a check off area is provided to indicate the Sanctuary as well as each of our Adult Bible Fellowship-ABF classes).
If a child needs the attention of a parent, one of the CM volunteers will locate where the parents are (according to the sign in sheet) and come to that area to advise them as to the situation.
