Initial Registration
At the first visit, the parent/guardian will be given a Child Information Card which includes the following information that needs to be completed: Child’s Name, Parent’s Names, Address, Phone Number, Birthday, Doctor, Allergies, Dietary Restrictions, Age/Department, Name of person(s) other than parent to whom the child may be released, Yes or No CWMC has permission to authorize emergency medical treatment, Parent/Guardian signature and date.

Weekly Sign-In
Each week, including that first visit, the parent/guardian will sign the child in on the attendance ‘Sign In Sheet’, which contains the following information: Child’s Age, Name, Parent/Guardian name, a comment area to record known allergies, etc., and where the parent/guardian will be (a check off area is provided to indicate the Sanctuary as well as each of our Adult Bible Fellowship-ABF classes). For Nursery and 1 year olds, there is a section also to identify if the child is nursing or takes a bottle.
Claim Tickets
The first few times attending, until we get to know the parent and child, each parent of children in Nursery – 3 yrs old will receive a ‘claim ticket’ which has a number on it that corresponds to the same number that is pinned onto the shirt/top of their child when they drop them off in the classroom. The claim ticket needs to be given to the teacher in order to pick the child up. A parent/guardian of children ages 4 – K can be given a claim ticket if they desire.
IDs and Systems for Locating Parents
After 3 visits, each child will receive a permanent child ID number.
These child ID numbers are used with the Visual Pager System we have in our sanctuary. When neeed, the child’s ID number is displayed on an LED screen that is located on the wall to the left of the stage in the sanctuary. The number remains lit (or flashes alternately with another number if another parent is needed) until the parent/guardian responds to their child’s number by going to the classroom to be with their child.
If a parent/guardian is not in the worship service held in the main sanctuary, and they are in an ABF classroom, or in the Venue, our contemporary worship, the teacher in the classroom will page our CE Director and give him/her the child’s name, state the problem, and where the parent can be located. The CE Director then goes to the appropriate area to get the parent and takes them to their child’s classroom.
